Listed below are brief descriptions of the District-specific forms that can be found on this website along with links to each of them. Please note that you must be Logged In with your Auxiliary Member ID (sometimes referred to as your Employee ID or employee number) in order to fill out and send a District-specific form. If you are not already logged in when you click on the link to the form, you will be prompted to log in before the form will be displayed.

Coastie Request

To schedule Coastie for an event in District 9 Western Regrion, fill out the Coastie Request Form. Click here for more information about Coastie.

Qualification Request

Flotilla Commanders may now complete an online Qualification Request Form in order to facilitate the updating of a member's qualifications with minimal paper work.

Member Training Intent to Teach

If you are planning to teach a course for Auxiliary members you can use the MT Intent to Teach form to request that your class be added to the Training Calendar.

WebEx Conference Request
